The launch of the Chiropractic Patient Incident Reporting and Learning System brings our Just Culture Position Statement to life. By offering a confidential, non-punitive system for reporting and learning from patient-safety incidents, CPiRLS creates space for honest reflection, open communication, and shared learning across the profession.

What is CPiRLS?

 

the Chiropractic Patient Incident Reporting & Learning System

CPiRLS is a confidential reporting and learning system, originally developed by the Royal College of Chiropractors in the United Kingdom in 2009. It supports regulated chiropractic professionals in reporting, analyzing, and learning from patient-safety incidents in a supportive, just-culture environment.

All Chiropractic Association of Alberta members now has access to CPiRLS through their member portal.

What Can Members Report?

An incident is any patient-safety event, error, accident, or deviation from the norm that:

 

Actually Happened

e.g. Patient fell from a treatment table

 

Nearly Happened (‘near miss’)

e.g. Patient nearly fell from a treatment table, but this was avoided

 

Has the Potential to Happen

e.g. Potential for patient to fall from the table noticed and action taken to reduce/remove risk

If it made you stop and think or discuss it with others - it’s worth reporting whether the outcome was positive or negative.

Every report helps the profession learn and improve patient safety.

How to Access CPiRLS

1. Log into your CAA Member Portal.

 

Access the secure portal using your member login and look to the section on CPiRLS for more information.

2. Enter the CPiRLS website.

 

Using the username and password provided on the member portal page, login to CPiRLS and access their secure reporting system.

3. Select incident type & submit a report

 

Once here, submit your report based on the type of incident that occurred. 

  • RED: Incident occurred
    • Example: Patient fell from treatment table.
  • AMBER: Near miss
    • Example: Patient almost fell, but was prevented.
  • GREEN: Potential risk
    • Example: Risk of fall identified and addressed.

Important: Reporting to CPiRLS does not replace your obligation to report patient safety incidents to your professional association or indemnity insurer.

Privacy & Confidentiality

 

All reports are submitted anonymously and kept strictly confidential. The process is designed to respect and protect the privacy of both patients and practitioners. Information shared through this program is never disclosed to regulators, government bodies, insurers, or partner organizations.

• Reports do not collect personal or identifying information

• Information is gathered to support learning and improvement.

• Findings are shared only in summary form to help identify system-wide insights

You can share incidents openly and with confidence, knowing your contribution helps improve care and safety for everyone. This program focuses on learning, growth, and prevention—not punishment.
