Yes, patients must receive a referral from a participating primary care provider within the Calgary Foothills or Red Deer Primary Care Networks.

Once you have obtained a referral from your primary care provider, the chiropractor will conduct an eligibility assessment to better understand your medical history and treatment goals. Based on this assessment and the program criteria, they will determine your eligibility for care.

Primary care providers can request a downloadable referral form here.

You do not need to reside within the participating PCN’s geographic catchment area. However, your referral and chiropractic care must be provided through a participating clinic and primary care provider within the program’s PCN network.

Eligible services provided through the program are publicly funded, meaning there is no additional out-of-pocket cost.

Eligible patients are able to receive one intake visit and seven follow-up visits at no cost based on the treatment plan created by the chiropractor.

Ask your primary care provider if they are participating in the Low Back on Track Pilot Program. You can also discover participating PCNs and chiropractic clinics through the information provided above.

During your intake visit, your chiropractor will discuss the evaluation component of the pilot program in detail. Participation in the evaluation portion is optional for all eligible patients.

The program will continue until government funding has been exhausted or a minimum of 350 patients have participated. Once either milestone has been reached, the evaluation study will help inform future applications for government funding to sustain the initiative.

If you do not have a primary care provider, you can visit a participating walk-in clinic within the Calgary Foothills or Red Deer Primary Care Networks. A physician or other eligible healthcare provider can assess your low back pain and determine whether a referral to the program is appropriate.

If you’re unsure where to access a participating walk-in clinic, visit the Calgary Foothills PCN or Red Deer PCN websites.
